Please be aware that any information provided through any part of our website is for guidance & informational purposes only and does not constitute professional advice.
At The Elegant Smile our services are guaranteed for a period of twelve months following placement, provided that the following conditions are met:
1.1 You have fully paid for the treatment and do not owe us any money for the treatment received.
1.2 The treatment received has not been damaged as a result of an accident or trauma.
1.3 You have followed all post-treatment maintenance recommendations made by our dentists.
1.4 No treatment is guaranteed for more than 1 year
It is very important that you give a full medical history and details of any
medication you take. Should these change in any way, it is very important for you to tell your Dentist. It is the patient’s responsibility to inform the clinic of any changes in either personal details and/or their medical history.
Certain treatments require completion of a written consent form. This is in
order to explain the treatment, aftercare and any risk to you thoroughly, before
any of these treatments are carried out.
You agree to indemnify, defend and hold harmless The Elegant Smile from any claim or demand, including reasonable attorneys’ fees, made by any third-party due to or arising out of your breach of these Terms and Conditions or the documents they incorporate by reference, or your violation of any law or the rights of a third-party.
In addition to other prohibitions as set forth in the Terms and Conditions, you are prohibited from using the site or its content:
(a) for any unlawful purpose;
(b) to solicit others to perform or participate in any unlawful acts;
(c) to violate any international, federal, provincial or state regulations, rules, laws, or local ordinances;
(d) to infringe upon or violate our intellectual property rights or the intellectual property rights of others;
(e) to harass, abuse, insult, harm, defame, slander, disparage, intimidate, or discriminate based on gender, sexual orientation, religion, ethnicity, race, age, national origin, or disability;
(f) to submit false or misleading information;
(g) to upload or transmit viruses or any other type of malicious code that will or may be used in any way that will affect the functionality or operation of the Service or of any related website, other websites, or the Internet; (h) to collect or track the personal information of others;
(i) to spam, phish, pharm, pretext, spider, crawl, or scrape;
(j) for anyobscene or immoral purpose;
or
(k) to interfere with or circumvent the security features of the Service or any related website, other websites, or the Internet. We reserve the right to terminate your use of the Service or any related website
for violating any of the prohibited uses.
7.1 Certain content, products and services available via our Service may
include materials from third-parties.
7.2 Third-party links on this site may direct you to third-party websites that are not affiliated with us. We are not responsible for examining or evaluating the content or accuracy and we do not warrant and will not have any liability or responsibility for any third-party materials or websites, or for any other materials, products, or services of third-parties.
Subject to the terms and conditions and relying upon the representations set forth herein; you commit to pay and/or cover all fees/costs that are due from the treatments you chose.
9.1 You understand that the novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. I further understand that COVID-19 is extremely contagious and is believed to spread by person-to-person contact; and, as a result health agencies recommend social distancing.
9.2 You understand that, even if you have been tested for COVID and received a negative test result, the tests in some cases may fail to detect the virus or you may have contracted COVID after the test. You understand that, if you have a COVID-19 infection, and even if you do not have any symptoms for the same, proceeding with this elective treatment can lead to a higher chance of complication.
9.3 You understand that possible exposure to COVID-19 before/during/after your treatment may result in the following: a positive COVID-19 diagnosis, extended quarantine/self-isolation, additional tests or hospitalization.
9.4 You understand that COVID-19 may cause additional risks, some or many of which may not currently be known at this time, in addition to the risk described herein, as well as those risks for the treatment.
10.1 We make appointments with the expectation that you will attend at the agreed date and time to undergo your treatment. If you change your mind or work/other commitments prevent you from attending, we ask you to provide us with the required notice (see below).
10.2 When booking your appointment, we will seek a non-refundable deposit from you which acts as confirmation of your commitment to attend your appointment.
10.3 If you are unable to attend your appointment for any reason, we shall
require a 48 hours’ notice from you before the scheduled start time of your appointment. Please note that Saturdays and Sundays will not be included in these time frames as well as Public holidays.
You acknowledge that all the booking information you have shared shall be treated as private and confidential and shall not be disclosed to unauthorized third parties. You hereby consent to the sharing of the said information collected for travel arrangements only with authorized parties. In addition to that you understand that we are in no way financially liable for any additional expenses that you may incur and you further release us from any responsibility in the event of an occurrence, injury, damage or loss in the course of your travel.
