Terms & Conditions

1. Program Coverage
The Elegant Care Program (“Program”) provides limited protection for eligible dental work performed by Provider. Coverage applies only under the conditions outlined herein.

Covered events include:

Repair or replacement of eligible veneers, crowns, or dental products that become chipped, cracked, or damaged through normal use.

Limited emergency assistance as defined by Provider.

This Program does not cover:

Loss of veneers, crowns, or products that are not saved and returned to Provider.

Damage caused by neglect, misuse, abuse, or intentional acts.

Damage resulting from failure to maintain proper oral hygiene or follow aftercare instructions.

Any issue not reported to Provider within 3 calendar days of occurrence.

Trauma or accidents unrelated to normal use.

Cosmetic dissatisfaction, shade changes, or elective adjustments.

2. Client Responsibilities

To remain eligible for Program benefits, Clients must:

Remain current on all Program fees and payments.

Report any covered issue within 3 calendar days of the incident.

Save and present any veneer, crown, or product that dislodges.

Follow all aftercare and maintenance instructions provided by Provider.

Attend all required maintenance and check-up appointments.

Refrain from misuse, including biting hard objects, using teeth as tools, or engaging in activities that risk damage.

Provider retains the sole and absolute right to determine whether Client has complied with these responsibilities. Failure to comply may result in denial or termination of coverage without refund.

3. Exclusions

This Program does not cover:

Lost, discarded, or unreturned veneers or crowns.

Damage from accidents, external trauma, or unsafe activities.

Issues arising from neglect, abuse, or misuse.

Any work performed by non-Provider practitioners.

Costs for travel, lodging, meals, or incidental expenses.

Normal wear and tear or minor cosmetic concerns.

4. Claims & Benefits

All claims must be submitted in writing with photos or documentation.

Provider determines, at its sole discretion, whether coverage applies.

If approved, repairs or replacements will be scheduled and performed by Provider exclusively.

Provider is not obligated to provide identical replacements; substitutions may be made at Provider’s discretion.

Coverage is non-transferable and applies only to the enrolled Client.

5. Limitation of Liability

Provider is not responsible for:

Any incidental, indirect, or consequential damages related to dental products or services.

Claims or costs associated with lost time, travel, inconvenience, or alternative care.

Services performed outside of Provider’s facilities or without Provider’s prior approval.

Program benefits are strictly limited to repair or replacement services as determined by Provider.

6. Fees & Termination

Program fees must be paid in full and on time to maintain active status.

Missed or late payments may result in immediate suspension or termination of coverage.

Provider may terminate Client’s enrollment at any time for non-compliance with these Terms, misuse of benefits, or inappropriate conduct.

No refunds are provided upon termination of Program participation.
